Desert Oasis Healthcare (DOHC), a member of Heritage Provider Network, and Wellth, an innovative Behavioral Economics company, announce a new collaboration to improve care plan adherence for individuals diagnosed with Type 2 Diabetes.
Wellth partners with healthcare providers and insurers to improve clinical outcomes and reduce costs of care for patients with chronic conditions. Using daily contextual nudges and rewards, Wellth has demonstrated 89% average daily adherence across all its members. Wellth members complete daily check-ins through Wellth’s easy-to-use smartphone app and receive financial incentives for verifying adherence to care plan behaviors such as taking medications or checking their blood sugar.
The new partnership between Desert Oasis Healthcare and Wellth will focus on improving adherence to medications and blood glucose monitoring while enhancing DOHC care team’s understanding of patient behaviors.
